Enrolment process

Children will need to turn five by 30 APRIL of the year they start school.

To enrol your child you need to fill out an enrolment form, available from the school office. You can also download a form from the link below.

Download Student Enrolment Form

The enrolment form is strictly confidential. Personal information about your child, as well as family members and others that provide care for your child, is collected so that Yarra Primary School (YPS) can register your child and allocate staff and resources to provide for educational and support needs. All staff at YPS and the Department of Education & Training (DET) are required by law to protect the information provided by this enrolment form.

If you are considering enrolment at our school, please do so as soon as possible. Early enrolment enables your child to be included in the important Prep Transition Program and also ensures YPS recieves appropiate resource allocations.

Along with the enrolment form, the school will need:

  • evidence of your child’s date of birth
  • names and addresses of the child and parents, guardians and carers
  • parents’ phone numbers (home, work, mobile) and email addresses
  • names and contact details of emergency contacts
  • doctor’s name and phone numbersand Medicare number
  • Immunisation Status Certificate
  • health and welfare information (for example, does your child have asthma, diabetes, allergies, poor eyesight or hearing, specific custody arrangements)
  • information about the language/s your child speaks and hears (to help the school build on your child's knowledge).

How to apply online for an Immunisation History Statement

Parents may request a statement of their child's immunisation history as recorded on the Australian Childhood Immunisation Register (Immunisation Register) by completing the form on the website. The statement will be mailed to the child's most recent address recorded on the Immunisation Register. You will need to enter your Medicare Card number. The Website address link is: Australian Childhood Immunisation Register.

Prep Transition

Yarra Primary School offers prospective Preps, enrolled for the following year, a successful transition program. Find out all you need to know about Prep transition and your child's first day of school.

School Uniform

The wearing of a school uniform is compulsory at Yarra Primary School. Many Parents find the uniform more durable than other clothing, it eliminates the problem of choice each morning and has the added advantage of helping a child feel they are part of the school.
