How to apply online for an Immunisation History Statement

Parents may request a statement of their child’s immunisation history as recorded on the Australian Childhood Immunisation Register (Immunisation Register) by completing the form on the website. The statement will be mailed to the child’s most recent address recorded on the Immunisation Register. You will need to enter your Medicare Card number. The Website address link is: Australian Childhood Immunisation Register.

When enrolling your child for school, a Certificate of Immunisation is required, stating that the child has received the immunisations against measles, mumps, diphtheria, polio and tetanus. If there is an outbreak of any of these diseases in the school, a child without the necessary immunisations can be excluded from school to prevent further spread throughout the community.

The certificate is available from the Australian Childhood Immunisation Register – Medicare Australia.

You can get a copy of your child’s immunisation history statement:

  • online
  • by requesting a statement to be sent in the mail
  • at your local Medicare office
  • by calling 1800 653 809
